Jennifer is a Type 2 diabetic with a history of straying from her prescribed diet. As a result, she frequently visits her ACO’s local Emergency Department. Prior to having Jiva, her ACO was dealing with siloed care information which resulted in Jennifer’s interdisciplinary care team having no idea when—or if—she was admitted to the hospital.
